How to make an appointment?
In the patient portal MijnGezondheid.net (MGn) you will be able to make an appointment online. You can select a date and time that is convenient for you.

You cannot make an online appointment in case of:

You may also call the clinic at +31 (0)40 – 711 67 30 and speak to the doctor’s assistant to schedule an appointment (see below “Speaking with the assistant”).

You are also able to request to be called back by the assistant at a time convenient to you. (see below “Request to be called back”)

In case of URGENT matters always call.

Cancelling an appointment
Cancelling an appointment can be done by calling the GP’s assistant.
Please let us know in time! You can cancel your appointment up to 2 hours in advance. If you do not cancel in time, we can charge € 15 euros.

Getting the results from a diagnostic test
You can request to be called back with the results (see below “Request to be called back”).
You can also call the clinic and request to be called back (see below “Speaking with the assistant”, option 4)

2. To cancel an appointment: you will be asked for your name, date of birth and details of the appointment
4. To get the results from a diagnostic test: you will be asked for your number and a time that is convenient to be called back
5. To make an appointment or another enquiry: you will be given the option to be called back or wait for the assistant to become available.

E-mail consultation

You will then be requested to log on using verification through DigiD + SMS. This gives access to the functions of MGn (Dutch health network) where you can request an eConsultation.
You can only use eConsultation in non-urgent matters that you have previously discussed with your doctor and that do not require a physical examination.

Request to be called back

Here you can indicate if it is regarding the result of a diagnostic test or if you want to speak with the assistant. You will be able to select a time slot that is most convenient for you to be called.

You can also request a call back by calling the clinic and selecting option 5 followed by 1.

Online Medical File – About MGn

Some of our online services use the online patient portal MijnGezondheid.net (MGn).

Through MGn you can set your preferences to receiving an email when there is a reply from your doctor on an eConsultation or to remind you of an upcoming appointment.  Select the Preferences button in MGn to set your preferences.

If you wish to use this service, please go to MGn and log in using your DigiD with SMS authentication. You can then activate your account by agreeing to the terms of use.

DigiD is a secure method developed by the Dutch government with which you can verify your identity on the internet. We strongly feel that your medical information should be secure, which is why we use the DigiD with SMS authentication.

If you don’t have a DigiD or DigiD with SMS authentication, you can request it at http://www.digid.nl/en.
Unfortunately this governmental DigiD site is still largely in Dutch.

If you need to cancel your registration, for example when you are moving or switching to another GP or pharmacy, you can use the form below:
